Baseline concussion testing will be August 10 beginning at 1:00 and continuing until 2:30. Everyone that is planning on being tested must complete the information attached in this email. Students should report to the high school commons.
Students need to click the Demographics link below and complete that information. They must then also use the QR code and answer the corresponding questions that are asked within the QR code.
Two important things to remember
- All information must be completed before arriving on August 10
- Information is to be completed by the student and not be the parent/guardian
